The surgery is performed under general anesthesia and typically lasts 2–3 hours, depending on the required incisions and associated procedures. Usually, you will stay overnight in the hospital.

During the surgery, part of the mammary gland is removed, and the remaining gland is repositioned to create a harmonious breast contour. The position of the areola is also elevated and, if necessary, its diameter can be reduced. Sometimes, the remaining breast tissue is insufficient to achieve the desired volume in the upper pole. In such cases, a breast implant or fat grafting (lipostructure or lipofilling) may be necessary.

Once you are back home, you will need to rest for at least the first 1 to 2 weeks, avoiding any exertion with your arms.

You will be required to wear a postoperative bra during the first few weeks, both day and night. In the first days, you may need assistance, although you should be able to gradually resume your regular daily activities within 2 to 3 weeks.

The scars will need to be treated according to medical instructions during the first few months to ensure they become as inconspicuous as possible.

Breast reduction will provide you with a breast volume more in harmony with your body, reducing the weight your back has to carry. Back discomfort and/or pain may improve due to the weight reduction, but you should be aware that any pre-existing damage will not be reversed.

Breast reduction surgery, or reduction mammaplasty, does not completely contraindicate breastfeeding; however, it depends on the type of incisions required to lift and reposition the breast tissue. Although breastfeeding is not strictly contraindicated, it is generally not recommended, as the mammary gland undergoes significant enlargement and subsequent atrophy during lactation, which could partially reverse the effects of the surgery. In some cases, the breasts may grow again, potentially affecting the symmetry and aesthetic outcome previously achieved.

Breast reduction, or reduction mammoplasty, can be combined with other breast procedures such as the placement of a breast implant or fat grafting to the breast tissue (lipostructure or breast lipofilling). Even when there is excess breast tissue, some patients prefer a rounder shape, especially in the upper pole. In these cases, a breast implant or fat grafting may be required to achieve a more natural-looking result.

Additionally, it can be combined with other body contouring procedures (such as liposuction in other areas or abdominoplasty), as well as facial procedures (such as facial lipofilling, blepharoplasty or eyelid surgery). However, the combination of procedures will depend on the complexity of the associated surgeries and the individual surgical risk of the patient.

Before the procedure, a full preoperative assessment is necessary to ensure your safety and optimal results. This typically includes:

• General blood tests (including coagulation profile)

• Electrocardiogram (ECG) – especially important if you’re over 40

• Chest X-ray – usually required if you are over 45 years old

• Mammogram and breast ultrasound – recommended for all patients undergoing breast surgery, to rule out any underlying breast conditions
